Hey! Its a bit tricky but you can try the following:

1. Deactivate collision of the gag, binder or other item (under physics object)
2. Position it where you want it to be
3. Connect the item to a Person using this plugin https://hub.virtamate.com/resources/parentholdlink-for-customunityassets.415/
4. Reactivate collision and hope that it works

There may be other ways to do it better. Did not play with attaching assets to people for a long time already. But this method worked for me pretty good back then
