The ring has collision enabled, and you can sometimes cause the physics system to freak out, especially if you're messing around with repositioning it in a mouth that's not open wide enough. You may in some cases need to turn off collisions to get it positioned, then perhaps have success with re-enabling collision after it's right where you want it.
The strap is also a CUA as I haven't yet cracked the problems related to making that a clothing item, such that it conforms to the head, without being overly mangled by morphs and also having the D-Rings continue to line up with the ends of the metal gag CUA. Making them both CUA seems to be the best setup I've got going yet, but I'll keep working on improving it.
I've sized this to work with the sizes of things I commonly insert into mouths, but your context may vary. You can scale it down a bit, but not much, without the strap either clipping into the neck or not lining up with the metal gag anymore. I've considered making an alternate metal object with the same width overall and just a smaller ring in the middle, but since I don't personally have a need for it, I haven't yet bothered. If someone wants to make a derivative work along those lines, I'm all for it.

- Set your target person's "Mouth Open Wide 2" morph to 1.4, providing sufficient room for this rather large device.
- Use Blasedust's CUAManager plugin and apply the included "Spider Gag with Strap" preset. That will load the gag and strap CUA assets and set their objects to be hold-linked to the person's head, such that bill_prime's "CUA Superglue (Hold Version)" will ensure they stay firmly attached.
- Alternatively, you can load the CUA assets, linking and positioning them yourself.
- You may also have to do some minor adjustments, depending on the active morphs you may have on your person that affect the head and mouth. Doing so can lead to physics explosions, so proceed with caution. I also suggest you save your results as another preset for the CUAManager plugin, so you can easily re-apply the various tweaked positioning again later if desired.
