Like for A.I chat roleplay with grok or Chatgpt or even character.ai, or if you want to use a cloned voice on elevenlabs to say some dirty stuff, how do you make it so it syncs with virtamate if you play it when tabbed out. I vaguely know it was possible at some point.

It's not... without big circumvolutions.
A game listen to audiosources. If you want to send sound, it's either microphone, or a loopback system from something that "simulates" a microphone.
More or less, if your app does not send data directly into the engine, you need plugins and external apps that will simulate a mic and then send it to the head of the character.
