The blood is wet skin overlay recoloured red on a hidden body. scaled down and posed.
The severed head is another body with the body shrunk bellow minimum settings =D
Did reverse for the main body. shrunken head =)

So you added a water drop texture to another body and made the body itself transparent to express the splash.
