Well, you can always be frontal about it. I've read threads of people with dolls discussing how they fit in with family members, and they range from the dolls being active participants with the couple, to be more reserved for one, semi-hidden, etc. It's like people who use VAM, veryy diverse in awareness, use and intentions.

Those are professionally shot photos, so don't expect them to look as good as they're shown. However, I am very impressed with the looks of my torsos, especially Aria that I linked. It only has the base skin paint but even that has some veins and skin pores to mimic real skin. I'm very curious to see the head I purchased, hasn't arrived yet.
