Horror/Monster Looks or Scenes?

Off the top of my head, @Syrinxo did a monster look and a scene, but they aren't that extreme:
There were a couple of other spooky scenes from over a year ago on the VAM Reddit sub, where the female was a little spooky, but not as extreme as what you are looking for (from what I recall). You can page through the directory and might be able to find them. However, those aren't VAR format and you'll need to import them into the appropriate directories.

In older versions of VAM, you'd sometimes get morph explosions that were pretty scary.
There were a couple of other spooky scenes from over a year ago on the VAM Reddit sub, where the female was a little spooky, but not as extreme as what you are looking for (from what I recall). Silent H Nurse - Looks -
thought i would try the silent H nurse , bit of a challenge modding the outfit to fit genesis 2 its not great as the head cloth textures colours don`t seem match the body textures . the head , dress ,hat and knife is from Rip van Winlkes...
