I want to create something like this

Any ideas how to do it ?
Is this plugin related type of stuff or can I do it easier somehow?
Any kind of help is welcome :)

You can make:
  1. Sign atom with text
  2. Image panel
  3. CUA
  4. Clothing item
1-3 need to be parent linked to the head to hold in place.
