I believe I left it at default, but cannot confirm for the moment. The penis is quite large and long in the scene this occurs, and the female is at an angle, so those may be important factors that make it an edge case. -

When Anal is ON, the penis is big and long and the person is at a bit of an angle, this shows up: Disabling Anal will remove it, which for my case is fine but wanted to share if there's something that could prevent it in future versions. -
